I got this file years ago off Ebay.
Think it might be of some use so I am putting it up for those who want it.

I had to download it twice. The first time the pdf file wouldn't open. 2nd time it worked just fine.

Probably the Hosting site has a problem. I did Test it . It did work for me .
I don't care if someone wants to place it some where more reliable.
I just looked for a site that wasn't. Full of popups and porn!
